Refund Request Form


A new refund request form must be submitted for each refund check.

Refunds from accounts with a Parent PLUS loan will be issued to the parent who signed for the loan, unless a signed note from the parent is provided stating that the refund may be made payable to the student. Please return this information to the Business Office at least one week prior to the refund check being issued
